ここに代行業者の記載がありますね。申請書はそちらが作成したのですか？

Yes. The new form confused me, so I had an agency prepare it. Contract, pay stubs, photos — it's all here.

はい。新しい用紙がわかりにくかったので、業者に作ってもらいました。契約書、給与明細、写真——全部ここに。

Your employment letter has your manager's signature. It also needs a signature from HR under the new requirements.

雇用証明書に上司のサインはありますね。新しい要件では、人事部のサインも必要です。

I may be wrong, but the agency told me the manager's signature was enough.

It was enough before January. The requirements changed with the form. They're listed on the back.

Yes. One more thing. Under "hometown," they've written "very remote." We need a place name.

That's from my notes! I gave them the name too. Getting home is complicated. So, apparently, is filling out this form.

Please write the name here, exactly as it appears in your previous application.

Of course. I paid them to save me a trip. Now I'm making two.

Everything else looks complete. Once you've corrected that, only HR's signature is missing.
